Please request all exams at least 2 business days in advance.

If you are taking your test at the main Logan campus please call us at (435) 797-9700 or (800) 233-2137, Mondays through Fridays between the hours of 8 and 5. If you are located near a USU site (please see map) contact the testing center there to schedule your exam. If you are not located in Utah or are unable to travel to/match schedules with the nearest USU site, fill out the form below.
