Affiliation Contracts

Once an intern has been accepted into the USU Dietetic Internship an agreement will be sent to each facility the intern plans to train at. Examples of our standard affiliation agreements are below for your review. If your facility has a different standard contract for student training, USU will be happy to honor it. Setting up and signing these contracts will not take place until the intern has been officially accepted into the USU Internship (after April of each year).
